Community Health Worker
The Community Health Worker (CHW) will be responsible for helping patients and their families to navigate and access community resources, various social services, and the healthcare system. The CHW will help patients adopt and maintain healthy behaviors by working individually with patients to identify and address barriers to care, set and achieve health goals, and reinforce information provided by other members of the care team in a culturally and linguistically appropriate, patient-centered manner. He or she facilitates communication between the patient and the care team, supporting the provider and the Nurse Care Manager through an integrated approach to care management and community outreach.
