Patient Portal

BCHC’s Patient Portal is safe, convenient and completely confidential.

With the Patient Portal you can:

• See your medical information
• Update your personal information
• Send a secure email message to BCHC staff
• Request or cancel an appointment
• Request a prescription refill

How to set up your BCHC Patient Portal Account

• Give the BCHC Front Desk your email address. We can set up a free email address for you, if you do not have one.
• Check your email for a message from BCHC with your temporary password.
• Sign in with your email address and temporary password. Your email address is your ID.
• Set up your own permanent password.
• If you forget your password, you can click on the “Forgot Your Password?” link to ask us to reset it.
• You can access the BCHC Patient Portal at: https://bchc.myezyaccess.com

Then you can save time and eliminate phone calls!

Use the PROFILE tab to update your address and phone number so we can always reach you.

Use the MEDICAL tab to see visit summaries and other medical information.

Use the MESSAGES tab to send your provider a secure message and receive a reply.

Use the APPOINTMENT tab to request an appointment or cancel an appointment.

Use the PRESCRIPTION tab to request medication refills.

Use the FAMILY ACCOUNT Option to manage multiple accounts for family members from one User ID.

The Patient Portal is confidential and safe. Only you can see your information. Do not share your password with anyone else so that you keep your privacy protected.

BCHC staff cannot give any computer support. We can only help to unlock your portal if you are unable to reset your password.
