The Behavior Health Case Manager will work with the Providers, Chief Clinical Officer, Behavior Health Director, Certified Recovery Specialist, BCHC Management Team and others to develop processes and systems that integrate each patient’s behavior health treatment plan and social services across all health care and social needs, within BCHC and between BCHC and all other health care providers and settings. Participate actively in BCHC’s mission to provide exceptional patient-centered primary and preventive healthcare for all residents of Berks County, regardless of economic status.
