Berks Community Health Center (BCHC) has received three Gold-level outpatient achievement awards from the American Heart Association, in recognition for its commitment to helping patients manage their health and reduce their risk of heart disease and stroke. The awards recognize a commitment to following the latest evidence- and science-based care guidelines.

BCHC received Gold recognition in the Target: BP™, Check. Change. Control. Cholesterol™, and Target: Type 2 Diabetes℠ programs. Each award recognizes proven dedication to following the latest evidence-based care guidelines to improve patient outcomes: 


Check. Change. Control. Cholesterol™ recognizes efforts to help patients better understand and manage high cholesterol, a key contributor to cardiovascular disease.

 

 


Target: BP™, presented jointly by the American Heart Association and the American Medical Association, honors BCHC’s success in supporting patients to achieve and maintain healthy blood pressure.

 

 


Target: Type 2 Diabetes℠ highlights BCHC’s commitment to supporting patients with diabetes, who face significantly higher risks of developing heart disease and stroke.
