2026 Fund A Need – Diabetic Care Program

At Picture Berks Healthy we shared a story that represent the experience of many of our patients:

*High blood sugar levels

*Serious chronic conditions including hypertension, neuropathy and kidney diseas

*Difficulty accessing healthcare due to long working hours at low wage jobs with no insurance.

BCHC offers a diabetes care program that provides glucose monitors, nutrition counseling and affordable medication. This program empowers patients who once had to choose between their health and their paycheck now have the knowledge, resources, and support to manage their conditions and live healthier, fuller lives — and that makes all the difference.

Help BCHC Fund our Diabetes Care Program!

$200 Provides one month of essential supplies for a patient in our care.

$500 Funds nutrition counseling and food support for an entire family.

$1,000 Supports care for multiple uninsured patients, including medication, testing, and education.

At PBH we made the call to fund this program and got a great response. But we are still $18,000 short of our goal. Any amount you can give will make a difference in our patients’ lives.
